Please use this form for making your Reservation. Enter the requested information below. You may submit the form online to alert us of your reservation, but please print it out and sign and date the bottom and mail it with your deposit. A 50% non-refundable deposit is required to reserve space. Also, send a copy of your vehicle registration and your driver's license. This is required if we need to show proof that we have permission to be in possession of your bike. Use one form per bike. Mail copies, form and deposit to Murphy's Transport, PO Box 365, Middleboro, MA 02346. Balance is due on agreed loading date.

Please list any other modifications to your bike such as exhaust extentions,
rake, non-stock handlebars, etc. Also list if your bike is longer than 8-foot.

COMMENTS:

THE MOTORCYCLE OWNER IS RESPONSIBLE FOR:
1. Preparing the motorcycle for shipment. All loose parts, protruding accessories, antennas, windshields, bags, etc. must be firmly secured to the motorcycle. The motorcycle ignition must remain unlocked, alarms and security devices must be off or disabled. No locks of any kind on forks or wheels. If your bike requires a key to start it, you must leave the key. There will be an additional inop fee of $50.00 for those bikes we have to push on the trailer.
2.Parts and/or accessories that are loose, torn, brittle, or have extensive wear. Please disclose on reservation form.
3.Articles/personal items left in vehicle unless we have been made aware.

MURPHY'S TRANSPORT is not liable for the following:
1.Faulty craftsmanship due to manufacturing defects (for example, defects in factory welds of tie-downs) or damage that results to the vehicle from the tie downs breaking or tearing.
2.Mechanical functions, exhaust assembly, undercarriage, alignment, suspension, tuning of engine, or alarms.
3.Damage caused by freezing of cooling system and/or batteries.
4.Damage to antennas that do not retract to within three inches of bike body.
5.Damage to any bike that cannot be driven on or off transporter under its own power. Brakes must be in good working order. A $50.00 service charge will be added to jump start or winch on or off transporter.
6.Damage unable to detect due to vehicle’s dirty condition.

General services to be expected for the care of your bike:
1. MURPHY'S TRANSPORT will prepare and load each bike individually and secure each bike with suitable strapping so as to protect the finish and original condition of the bike at all times.
2.The bikes are loaded according to expected delivery times. They remain loaded and locked in the trailer for security reasons whenever delivery is delayed. It is the customer’s responsibility to contact MURPHY'S TRANSPORT in case you will be delayed.
3.Any bike found to have narcotics, illegal drugs or weapons on or in the bike will be turned over to officials without liability to MURPHY'S TRANSPORT and without refund to the bike owner. MURPHY'S TRANSPORT will assist law enforcement with any such investigations at any time.
